Headache (a summary of the standard for a family doctor).

Tension headache is not observed. The pain of medium intensity, with no voltage increases.

Migraines are recurrent attacks of moderate to severe, typically unilateral, throbbing pain in nature with nausea and / or vomiting, sometimes celebrated the aura; the pain increases with physical exertion, there is light. Duration of attack is always 4-72 hours.

Migraine in children, see 'Headache'. Differences: it is often two-sided, 30-minute-48 hours. :: Compressing, compress, usually bilateral headache without nausea and vomiting. Duration: minutes-a-days. Increased light and Aura: stereotyped, often visual disturbances before the migraine attack, an hour will begin immediately phase of headache.

Headache due to drug abuse: Chronic headache> 3 dthreeys a week, most of thedayagainst theuseof drugs forheadaches, protivomigrenoznyh drugs or>5 unitfive of coffeea day.
Clusterheadache: paroxysmal in nature,expressed or tearingthrobbing, unilateral around the eyeor temple area, lasting15-180 minutes, forcingthe patientto move(the patientdoes not find a place of pain) occurs in the underrm ofclusters within a few weeks.

Diagnosis

The family doctor should pay attention to the following danger signs:

Dangerous Symptoms Differ-th diagnosis
For the first time encountered bouts of headache in people older than 50 years, brain tumor, arteritis temporalis.
The first attacks of migraine over the age of 40 years, brain tumor
Headache in a child younger than 6 years old with a brain tumor, hydrocephalus
Elderly patients with pain in the temple arteritis temporalis
Pregnant with a headache of unclear cause pre-eclampsia
Increasing pain after an injury sub / epidural hematoma
Severe headache against the background of very high pressure Malignant hypertension
Acutely emerged, very severe pain meningitis, stroke, subarachnoid hemorrhage.
Headache with fever (and of consciousness), meningitis
Meningeal symptoms / neurological symmetry we meningitis, brain tumor
Headache with signs of increased intracranial pressure, brain tumor
Symptoms of lateralization of neurologic symptoms of brain tumor
Vomiting in the morning, vomiting not associated with headache a brain tumor.
Changes in personality traits / loss of school success with a brain tumor
Migrainou's aura is always on the same side as the brain tumor.

The family doctor should specify:

  • The nature, severity, time of occurrence (a day), location, duration of headache;
  • Family predisposition was noted whether a similar pain before;
  • Accompanying symptoms (nausea, vomiting), the aura or prodromal symptoms;
  • Aggravating factors (hormonal factor or factors as alcohol or stress);
  • (self-) medication (pain relievers) and the use of caffeinated products (cola, koffie, thee, ice-tea, chocolade);
  • the patient's behavior during the attack (preferred bed rest or vice versa, does not find a place);
  • The degree of pain, limitation in daily life, at work, at home.

Physical examination and additional methods

Not shown in the complaints, pointing to the tension headache, migraine, pain, the abuse of drugs. Sometimes shown to calm the patient.
When a cluster headache can be seen accompanying this suffering symptom.

Score

  • Set the diagnosis of tension headaches, migraine, cluster on the basis of pain complaints.

Informing the patient and non-pharmacological treatment

Tension headache increased sensitivity of muscle skull and the insertion of their tendons to the skull. Rational allocation of the burden on them often gives a positive effect.

Migraine established control over pain. Avoid provoking factors (has a limited effect.) Can be effective methods of relaxation and cognitive therapy.

Migraine in children. Consolation-educating parents and children about the short duration of the disease. Recommendations of recovery sleep during an attack of a compliance piece of mind.

Headache abusing drugs. The reason for this excessive use of drugs against headaches or caffeine. I can develop addictive. Tip: cross-sectional end the use of funds.
Cluster pain. Cause of the unknown. Avoid provoking factors during the cluster period .. 'Hereditary predisposition' and 'alternating expansion and contraction of vessels with pulsating pain as a result. " The goal of treatment

  • Migraine, emerged against the background to the use of birth control - try another.
Migraine during the stop of the week: try to continue taking the contraceptive.

Migraine in children is usually enough paracetamol in sporadic attacks.

Cluster pain treatment during an attack of 100% oxygen, 7-10 liter through a mask for 15 minutes, or 6 mg sumatriptan subcutaneously, or 20 mg as a spray for the nose (max two times) ..

  • Rate control for tension headaches treatment effect in 2-6 weeks.
  • When a migraine - the control effect after 2-3 episodes, with preventive treatment for two weeks for side effects and three months for efficacy.
  • Chronic pain keeps in touch with the patient.
  • Be cautious when renewing prescriptions for painkillers, triptans and ergotamine on the phone.
  • Encourage frequent communication (including telephone) to patients who stopped taking the means by which they abused.

Coronary Angiography Guidelines

In patients with multivessel lesions measuring the relative coronary flow reserve improves the outcome of coronary stenting. FAME results of the study

Coronary angiography (CAG) remains the gold standard in determining the place of percutaneous coronary intervention (PCI). Nevertheless, in multivessel lesion definition hemodynamically significant stenosis, i.e., lesions of a coronary arteries, causing myocardial ischemia, often causes difficulties. In a study of FAME (Fractional Flow Reserve versus Angiography for Multivessel Evaluation) was shown that the determination of the relative (partial), coronary flow reserve (fractional flow reserve; FFR) with stenting only hemodynamically significant stenosis by approximately 30% reduces the risk of major adverse cardiac events within one year compared with the standard strategy of PCI of all anatomically significant stenoses. In an article published in JACC for July 13, 2010, the researchers analyzed the results of 2-year surveillance study participants.

Methods and progress in the study.
The study included patients with multivessel coronary heart disease (CHD) in the presence of anatomic stenosis ≥ 50%, at least two major epicardial coronary arteries. Patients with acute myocardial infarction (MI) with a rise or without ST segment, elevation could be included within the study, but not earlier than five days from the onset of the disease. It excluded were patients with stenosis of the left coronary artery trunk was transferred coronary bypass surgery, cardiogenic shock, severe tortuosity or calcification of the coronary arteries, the life expectancy of less than two years.

The standard approach (Group CAG) PCI all anatomical stenosis (≥ 50%), was carried out according to traditional methods. At a physiological approach (group FFR) first for each stenosis by coronary catheter for pressure measurement (Radi Medical Systems, Sweden) was determined by FFR. This indicator was measured during maximal hyperemia induced by administration of adenosine (140 mcg / kg / min) through a central vein, and was calculated as the ratio of mean pressure distal to coronary stenosis for mean pressure in the aorta. Only stented lesions with FFR ≤ 0,80. All participants were implanted stents coated with antiproliferative drugs. All patients at least one year after PCI received aspirin and clopidogrel.
The primary endpoint of the study served as a combination of major adverse cardiac events (death, MI, repeat revascularization) within one year after PCI.

Secondary endpoints - death / MI, and major adverse cardiac events and their individual components up to two years of observation, duration of intervention, the number of the used contrast agent, functional class of angina at 1 and two years, the number of antianginal drugs, cost-effectiveness of treatment.

Results.
Enrollment of patients conducted from January 2006 through September 2007 in 20 centers in the U.S. and Europe. Included 1,005 patients (average age - 64 years; men - almost 75%, MI registered - 37%; transferred PCI - 27%, diabetes - 25%). Patients with acute myocardial infarction were not. CAG group (n = 496) and FFR (n = 509) was comparable in baseline characteristics, including the number of anatomic stenosis, the number of diseased arteries, the severity and extent of lesions. Mean left ventricular ejection fraction was 57%.

In a total 2,415 study participants implanted stents are coated implants - in 96, 9% cases. Under physiological approach, FFR was measured in 94% of all stenoses, and for 874 (63%) lesions it was 0.80 or less. Once these lesions were stented. In the remaining 513 (37%) cases, FFR was more than 0.80, and these stenoses did not operate.

As a result, the group of CAG stents found significantly greater than in the FFR: 2,7 ± 1,2 vs. 1,9 ± 1,3 (p <0,001). Length PCIwassimilar in both groups (70and71 minutes respectively, p = 0.51). However, in the FFR, group was used by some smaller amount of contrast medium (302 ± 127ml vs 272± 133ml, p <0,001). Lengthof hospital stay was almost the same: 3,7 ± 3,5 days after the group of CAGagainst 3,4 ± 3,3 days after the group of FFR(p= 0,05).2-yearobservation was completed in 93.6% of the participants. Overall mortality in groups of CAGand the FFR was 3.8% (19 cases)and 2.6% (13 cases), respectively (p = 0,25),the frequency of MI - 9, 9% (n = 49) and 6,1 % (n = 31) (p= 0.03).The combination of death and MIwassignificantly lower in the: 8, 4% against 12, 9% in the group of CAG(p= 0,02).The frequency of repeated revascularization was comparable to the groups: 12, 7% (n = 64) in the CAG group and 10, 6% (n = 54) in a group FFR (p= 0.30).Numberof major adverse cardiac event was not significantly lower at a physiological approach: 22, 4% in the group of CAGagainst 17.9% in the FFR (p= 0.08).The number of patients without angina, and was similar: 76% and 80% respectively (p = 0.16).
coronary angiography procedure view

Over 2 years of observation in the FFR group recorded only one MI (0.2%) associated with non-operated stenosis, and at most 16 (3.2%) reinnervations in the not operated lesions.

Conclusions.

Patients multivessel CHD who underwent PCI only hemodynamically important (based upon the definition of FFR) stenosis within two years remained preferable outcome than using a standard strategy of stenting of anatomically significant stenoses.

The probable cause of greater efficiency PCI-based FFR, according to the researchers, should be considered a justified use of stents in comparable ischaemic effect. PCI all angiographic stenoses, regardless of their "ischaemic potential, can lead to additional risk of stent thrombosis or restenosis, which outweighs the risk of ischemic events without stent implantation at the site of hemodynamically insignificant stenoses.

Scientists believe that the PCI based on FFR can lead to better clinical outcomes compared with conservative therapy than it was in the study COURAGE, or coronary artery bypass grafting, as shown in the study SYNTAX. This is even more likely that the fragment COURAGE study demonstrated reduced risk of death and MI in patients with stable CHD with greater freedom from myocardial ischemia.

Systemic Lupus Erythematosus Guidelines

For the saturation of vitamin D in patients with Systemic Lupus, Erythematosus need a dose of 2000 IU.

Five of six patients with Systemic Lupus Erythematosus (SLE), taken daily to 2000 IU of vitamin D reached a level of 25-hydroxyvitamin D in plasma of 30 ng / ml or more. Study proved no National Institutes of Health.

18 people with the initial average level of 25-hydroxyvitamin D 13,3 ng / ml were divided into three equal groups: six patients received 800 IU vitamin D once a day, six - to 2000, IU per day and the rest - to 4000 IU.

After 12 weeks, 67% (four patients) in the first group, 83% (five people) with the second, and 67% (four) with a third group of indicators reached 30 ng / ml and above.

In the third group 33% (two patients), level 26 (OH) D pevysil 40 ng / ml, which was not observed in any of the subjects who received doses lower.
nice guidelines systemic lupus erythematosus

These results are important, said during an interview after the conference, the author of the experiment, Dr. Diane Kamen, a rheumatologist at the Medical University of South Carolina in Charleston, because although there is growing awareness that such high doses of vitamin D needed to restore the levels of 25 (OH) D in patients with autoimmune diseases in rheumatology literature, until now, included recommendations for doses of 600-800 IU / day.

18 patients were selected from the black population that lives on the sea islands of South Carolina and Georgia, Gullah izvestnog the population whose nablyudaetsya greater incidence of lupus.

Previous research among Gullah nation found that 43% of the 187 subjects had a level of 25 (OH) D below 10 ng / ml in some level is not defined at all. Low levels correlated with a high index of SLE activity and the content of antibodies to double-stranded DNA, said Dr. Kamen.

Although 2000 IU per day raised the level of 26 (OH) D in most patients, the rheumatology community, there is a debate about whether there should be greater than the target level of suffering in foreign currency.

"We know that 30 mg / ml is the minimum that considered normal," - she said, noting below this level may begin secondary hyperparathyroidism.

Giant cell arteritis is often overlooked when it is too late (Rheumatology and Immunology)

Giant cell arteritis Considered emergency drills in rheumatology, but it is not always diagnosed before irreversible changes occur.

Giant cell arteritis * (HCA) - primarily a disease of older people - is a systemic vasculitis mainly characterized by inflammation of the large and middle cerebral arteries, especially the blood vessels at the temples. Although genetic, infectious and autoimmune factors play in the pathogenesis of the disease known role - its cause remains unsolved. If the disease is not diagnosed and treated - it can lead to irreversible vision loss as a result of necrotic posterior ciliary branches of the ophthalmic artery.

The key to early recognition of the HCA is in the doctor's awareness of the characteristic ocular and systemic symptoms of the disease, their consideration in the differential diagnosis when an elderly patient arrives with several non-specific complaints, said Dr. A. Stephen A. Paget, program director of the arthritis center at Cornell University and the Multipurpose Arthritis Center and diseases of the locomotor apparatus at the Hospital for Special Surgery ** in New York. Because of the variability of symptoms, however - "Patients with HCA can walk to many physicians in key, but not limited to, internist, ophthalmologist, neurologist, otolaryngologist, dentist and rheumatologist yet diagnosed - he says, noticing, that diagnosis.

Dr. Stephen answered a few questions for the publication of Internal Medicine News about the diagnosis and treatment of HCA.

Q: How common is the HCA, and what its major symptoms?

Dr. Paget: HCA often sochetaetsya with polymyalgia rheumatica. Half of the patients have symmetric proximal pain, stiffness, and 5-10% with polymyalgia rheumatica year after it began to develop temporal arteritis.

Painting HCA also includes other symptoms: severe and persistent headaches, pain in the jaw during meals, sore tongue, a sudden loss of vision (passing or permanent), flashing lights before your eyes, fever, weakness and weight loss.

Q: What are the diagnostic criteria for the HCA?

Dr. Paget: Criteria of the American College of Rheumatology in 1990 require three pinktov of the following: age 50 years or older, the appearance of localized headache, sensitivity of temporal artery on palpation or weakened its ripple, ESR 50 mm / hour or more and / or; the positive results of temporal artery biopsy.

Q: What are the current standard treatments for this condition perimenopausal basic antirheumatic drugs that modify the course of the disease (BPRP)?

Dr. Paget: Prednisolone is still an excellent drug against HCA and should be appointed immediately after the clinical diagnosis pending the results of the biopsy. Moreover, the characteristic dramatic improvement within 1-3 days after the start of steroid therapy helps to confirm the diagnosis. Methotrexate and azathioprine can izpolzovatsya in combination with steroids, but they should not be prescribed initially.

Question: What is the prognosis for patients who were examined and treated in time?

Dr. Paget: HCA, usually last a year or two, after which patients can do without drugs. The probability of recurrence is very small.

* Giant cell (giganto kletochny), temporal (temporal) arteritis (GTA), or Horton's disease - a systemic disease almost exclusively of elderly or old age, characterized by granulomatous inflammation gigantnokletochnym predominantly extra-and intracranial vessels affects the arteries elastic and the mixed type, usually one or more branches of the carotid arteries. In particular, often suffer temporal artery.

The effectiveness of statins in question

USA - Very large-scale clinical trial, 2008 JUPITER, designed to prove the reduction in mortality from taking statins for primary prevention, covering 75% of the U.S. population who used these drugs, passed with such strong remarks that it is hardly tenable.

In the first critical statement about the 2008 JUPITER *, Dr. Michel de Lorgeril of the University Joseph Fourier and CNRS, Grenoble and colleagues analyzed the methods and results of this very large study where that rosuvastatin demonstrated a clear reduction in mortality in patients with low cardiovascular risk, which not evidence of coronary heart disease. These unfounded conclusions, immediately caused controversy, "no doubt prompted many healthy people without high-cholesterol levels, a long-term statin treatment," - says Dr. de Lorgeril.

* Justification for the Use of Statins in Primary Prevention - Rationale for the use of statins in primary prevention.
First among the main drawbacks JUPITER called it an prompt completion, and apparently without adequate justification, in accordance with their own same study protocol. Dr. de Lorgeril and his colleagues criticized the early termination of funding, mainly by commercial organizations, tend to impose bias that exaggerated the benefits and cover long-term effects of treatment. They also suggested that the study was terminated as soon as the mortality curves between the group receiving statin and control groups began to converge, indicating that significant differences between groups may have disappeared, if the study was terminated prematurely, they write.

The main pretext for the termination of JUPITER was "unequivocal reduction in cardiovascular mortality," with statin therapy. Nevertheless, in a published report, the authors do not call JUPITER data on cardiovascular mortality Readers should draw a conclusion about these results by extrapolating data from tables. By careful study, however, the data show an identical cardiovascular mortality between the study groups.

Approved the new international diagnostic criteria for rheumatoid arthritis

International criteria for diagnosis of RA for the first time changed since publication in 1987. The new version eliminated the expert's insufficient sensitivity of the criteria for early manifestations of the disease.

Consensus was achieved by the joint working group of the American College of Rheumatology (ACR, the former American Rheumatism Association) and the European League Against Rheumatism (EULAR).

Criteria published in the official journals of both organizations - Arthritis and Rheumatism (2010; 62:2569-81) and the Annals of the Rheumatic Diseases (2010; 69:1580-8), respectively, were created in three stages over two years.

Work focused on identifying the patients with recently appeared undifferentiated synovitis factors that best distinguishing those who have a high risk of developing persistent and / or erosive forms of disease.

The new scale diagnostic assigns points as follows:

  • One of the affected "big joints (shoulders, elbows, hips, knees and ankles) receives a score of 0, 2.10 part of large joints gets 1 point.
  • Involvement of 1-3 "small joints (phalanx, proximal interphalangeal, from second to fifth metatarsophalangeal joints, interphalangeal sustavoa fingers and wrist) gets 2 points, regardless of the involvement of large joint's involvement of small joints of 4.10 gets just 3 points.
  • The interest of more than 10 joints, including at least one large, gets 5 points.
  • And a negative rheumatoid factor (RF) and a negative test for autoantibodies (anti-citrullin protein antibody test, ACPA) together receive 0 points, while the "weakly positive" RF or ACPA (defined as less than three times the upper limit standards) get 2 points. For either of the two indicators is charged 3 points.
  • Normal CRP and ESR receive a "0," while at least one positive test gets 1 point.
  • Duration of the presence of symptoms less than six weeks gets a score of 0, the duration of six weeks or more - 1 point.
Scores of 6 or more of 10 qualifies as an established diagnosis of RA.

Dr. Eric L. Matteson, a professor of medicine in the department of rheumatology at the Mayo Clinic in Rochester, Minn. did not participate in the study, commenting in an interview with the online edition of Internal Medicine News new standards said: "The main useful feature is that the new criteria do not require the presence of several affected joints for the decision of early diagnosis of inflammatory rheumatoid arthritis.

Moreover, the patient can gain 6 points and no plural inflammation of the joints, in accordance with the new approach.

The authors noted that the symmetry of the lesion is not a criterion for diagnosis, as this symptom does not show the value at any stage of development criteria. Nevertheless, they wrote: "... inevitably greater the number of involved joints - probability of bilateral lesions.

When Dr. Matteson asked what are the drawbacks is a new criterion - he pointed to a lack of awareness (the authors) on extra particular RA components, which can also occur in the early stages of the disease.

"When they are, they can be very useful for the detection of the disease, and they are important markers of disease severity and symptoms and need treatment," - he said.

The new guidelines are also available biomarkers to assess the efficiency of a treatment, he added.

Insulin Pump: Not everything is as

According to the Office for quality control of foodstuffs and medicine's USA (FDA), have been increasing reports of unstable insulin pumps - miniature devices, responsible for the timely flow of insulin into the body of patients with diabetes who have such devices on the belt.

"Problems arise with instruments issued by different manufacturers - said the representative of the FDA. - At present, we are aware of 18 cases of revocation of devices over a period of five years."

On Friday, March 5, 2010, FDA plans to convene an advisory committee to discuss measures to be taken to "reduce the risk to patients of recalls of devices."

Insulin pumps are used primarily patients with diabetes mellitus type 1, in which the pancreas does not produce or produces insufficient insulin for utilization of sugar entering the body with food. Such patients must take insulin every day - in the form of injections or using an insulin pump.

A more common type two diabetes, often associated with obesity and usually develops in older people treated with saharoponizhayuschih preparations, although in some cases also require insulin.

According to the FDA, the popularity of insulin pumps in patients with type one diabetes is increasing: in 2002 in the U.S., there were 130 thousand users of such devices, then by 2007, their number reached 375 thousand.

Manufacturers are required to notify the FDA about the problems with insulin pumps. In turn, the Office conducted an analysis of side effects associated with the use of devices, identifying with the October 1, 2006 to September 30, 2009 almost 17 thousand of such cases. Each of them does not necessarily mean that the problem was caused by an insulin pump, but it was an occasion for a more thorough investigation.

Even if the device is working properly, problems can be caused by the fact that the patients themselves because of ignorance or negligence, use it correctly. Of the 17 thousand cases per 12 thousand injured patients (unchanged blood-glucose levels), and in 310 cases of problems with the device caused the death of patients.

Moreover, in 225 cases of 310 problems was described as "unknown," although in many cases, the device has never returned to the manufacturer for examination or repair. In 41 cases, although the problem with a device called the "unknown" cause of death of the patient becomes too high or as well low blood-glucose levels, which indicates a possible malfunction in the device.

Cardiovascular drugs and medicinal herbs - bad company

A recent study, intake of cardiovascular drugs with herbs can do more harm than good.
Some popular herbs, such as ginkgo biloba and St. John's worth may intensify the effects of several drugs, appointed by the elderly for the treatment of cardiovascular diseases.

Meanwhile, it is estimated that more than 15 million Americans daily consume certain herbs, believing that thereby strengthening their health.

For example, some religiously believe that St. John's Wort improves sleep and helps fight depression. Others believe that herbs can be trusted to care and serious illnesses such as cancer, and they do not mind the lack of clinical evidence of this.

"Interaction between certain herbs and drugs can lead to life-threatening condition - explains Dr. Arshad Jahangir from the branch of the Mayo Clinic in Arizona. - May be disrupted blood clotting, increases the activity of hepatic enzymes, there may be changes on the ECG.

The study is published in the Journal of the American College of Cardiology "(Journal of the American College of Cardiology).

Levels of CA125, HE4, mezotelina increase over 3 years prior to diagnosis of ovarian cancer

A study conducted by American scientists have suggested that serum level of CA125, the human epididymis protein 4 (HE4), and mesothelin increase over three years prior to diagnosis of ovarian cancer, but significantly increased only during the year prior to diagnosis.

"The duration of the preclinical period, observed in this study gives some hope that the time period before diagnosis, is 3 years or more, can be obtained if the identified correct circulating protein", - said Garnet Anderson (Garnet Anderson) (Fred Hutchinson Cancer Center, Seattle, Washington, USA) and colleagues.

Scientists have used immunological tests to determine the concentration of CA125, HE4, mesothelin, B7-H4, decoy receptor 3, spondin-2 proteins in stored serum samples 34 patients with subsequent diagnosis of ovarian cancer and 70 control patients who participated in himiopreventivnom cancer research lungs

Serum concentrations of CA125, HE4, and mesothelin began to grow somewhat in ovarian cancer patients, approximately three years before diagnosis, but reached high levels only in the final years before diagnosis.

Regression analysis showed that women with CA125 levels 53 U / ml of a 1-year probability of ovarian cancer is approximately 1.4 times higher than that in women with levels 16 U / ml. Further analysis showed that HE4, mesothelin, and spondin-2 are also associated with a significantly increased risk of ovarian cancer, with risk ratios ranging from 1.35 to 1.58.

Garnet protects against breast cancer

Anti-aromatase grenades reduce the risk of breast cancer, according to research Division of Tumor Cell Biology at Beckman Research Institute and the American Association for Cancer Research, USA. The components that make up the grenade, inhibit the production of the hormone estrogen, thereby preventing reproduction of cancer cells in breast and blocking the formation of tumors, researchers have identified. Fruit has a high polyphenol and antioxidant that also have a positive effect on the tumor. In addition, ellagic acid in pomegranate inhibits the action of aromatase - an enzyme that converts an androgen into estrogen. Aromatase plays a key role in breast carcinogenesis. Thus, the pomegranate represents a huge potential for protection against dangerous diseases, are confident oncologists.

Doctors decided to evaluate the possibilities grenade in the suppression of aromatase, studied 10 ellagitannin-containing components in its composition. It was proven that these natural compounds can successfully prevent the formation of malignant tumors in the breast. In particular, in urolitin produced metabolites of ellagic acid and related compounds, significantly inhibits the growth of cancer cells. However, doctors say that ellagitanniny not well absorbed into the blood, so in the future to find a way to use pomegranate juice or fruit, a separate connection for health and cancer prevention.

Defective cell culture questioned the results of cancer research

Scientists from the Netherlands, questioned the results of numerous scientific studies of cancer, according to MedPage Today. According to experts at the University of Erasmus of Rotterdam (Erasmus University Rotterdam), for these studies were used inappropriate cell culture. The researchers studied the genome of 13 of the 14 currently used in cell cultures of esophageal adenocarcinoma. It turned out that the three cultures (SEG-1, BIC-1 and SK-GT-5) contained a cell tumor of the lung, intestine and stomach.

According to the Dutch experts, the results of this study formed the basis of clinical trials of drugs. In addition, the results of this work were written over a hundred scientific articles, as well as eleven issued patents and three grants for further research in this area.

In addition, scientists from Rotterdam consider it necessary to revise the rules of appointment of the drug, sorafenib, used in the treatment of esophageal cancer, since studies of its effectiveness have been used in cells of other types of tumors.

Representative of the British Health Protection Agency (Health Protection Agency), which owns a large collection of cell cultures, said that the problem of the use of inappropriate cell research arose more than 20 years ago. He noted that the organization's website published a growing list of cell cultures, certain incorrect or contaminated by cells of another type.

Insecurity of early ventricular repolarization syndrome is confirmed.

For decades, the syndrome of early ventricular repolarization (SRRZH) is regarded as an innocent electrocardiographic phenomenon. Only recently it was shown that the recovery point J in leads other than V1-V3 (especially in leads from the bottom or side walls) is accompanied by an increased risk of ventricular fibrillation. However, the prognostic significance of SRRZH in the general population is not established. Jani T. Tikkanen et al. conducted a retrospective study of a population of middle age, a representative of the general population of Finland, to assess the prognostic value of SRRZH in the bottom and side-lead standard electrocardiogram (ECG) for death from cardiac causes, cardiac arrhythmias, and from any cause.

Methods and progress in the study.
A population study was selected among the study participant's Coronary Heart Disease Study, in which from 1966 to 1972 was included 10,957 persons aged 30 to 59 years from different regions of Finland. All participants, in addition to clinical examination, performed the measurement of body mass index, blood cholesterol was determined and recorded by standard ECG. All participants completed a questionnaire about lifestyle, established disease and taking medications. All ECGs were re-analyzed for SRRZH. Not subject to interpretation found only 93 ECGs.

Analysis of the ECG recorded at 50 mm / s, conducted by five doctors. Diagnosis SRRZH in the lower (II, III, aVF) and lateral (I, aVL, V4-V6) leads placed during the second revision of the ECG by a consensus of researchers. SRRZH divided by the degree of recovery points of J (≥ 0,1 mV or> 0. 2mV )and form connections of the QRS complex with segment ST (in the form of notches on the knee of S-wave, or a smooth transition). In addition, the criteria Sokolov - Lyons determined left ventricular hypertrophy (LVH), according to the formula Bazetta - corrected interval QT. In some individuals to re-register the ECG was carried out in 1973-1976 years. The primary end point of the study was death from cardiac causes, secondary endpoints - death from any cause and death from arrhythmia before the end of 2007, deaths and its cause is established according to national statistical databases. Death from arrhythmia approved by an independent committee of cardiologists, un informed about ECG data, the analysis of death certificates and hospital medical records. Relationship between SRRZH and clinical outcome was determined in the proportional hazard's model of Cox with correction for sex, age, systolic blood pressure, body mass index, heart rate (HR), smoking, left ventricular hypertrophy and signs of coronary heart disease (CHD) on the ECG with Minnesota code. Results. Study population was 10 864 people (average age - 44 ±8 years, men - 52,4%). SRRZH of them was found in 630 subjects (5.8%). In the lower leads SRRZH detected in 3.5%of cases (384participants, men - 70%)in the lateral leads - in 2, 4% of cases (262 participants, men - 58%)in the lower and lateral leads - to 01% of cases (16 participants). Elevation point t J> 0,2 mV were observed in 36 participants (0.3%) in the bottom leads and have 31participants (0.3%) in lateral ECG leads. Of 542 subjects with SRRZH on the initial electrocardiogram during repeated ECG recording (on average- after 5years) it was preserved in 443 (81,7%).

Participants with lower SRRZH leads in comparison with persons without SRRZH were more often men; smokers had a lower heart rate and body mass index, a shorter interval QT, but most of QRS duration and more frequent signs of CHD. In the presence of SRRZH in lateral leads more often showed signs of LVH.

During the observation period (30 ± 11 years) died 6133 study participants (56.5%). Deaths from cardiac causes accounted for 32.1% of all deaths. Of these, 40.4% of cases were considered sudden death from arrhythmia. In multivariate analysis of a person with a lower SRRZH lead had an increased risk of death from cardiac causes (corrected relative risk [RR] - 1.28, 95% CI 1,04-1,59, p = 0.03) and from arrhythmia ( RR - 1.43; 1,06-1,94, p = 0.03) but not total mortality (RR - 1.10; 0,97-1,26, p = 0.15). Among participants with elevation point J> 0, 2mV in the lower leads marked increase in risk of death from any cause (RR- 1.54, p = 0.03), from cardiac causes (RR- 2.98, p <0,001) and of arrhythmia (OR - 2.92, p = 0.01). Survival curves of these parties and people without SRRZH began to separate after about 15 years and continued to diverge in the future. Almost half (47.2%) participants with the elevation point of J> 0, 2mV in the lower leads died from cardiac causes, while this cause of death was noted only one in five subjects (17.2%) without SRRZH.
Prognostic value SRRZH in lateral leads was at the border level of statistical significance for mortality from heart failure and any cause, but was unreliable for predicting death from arrhythmia. A secondary analysis of the risk of death from cardiac causes was higher in patients with long intervals of QT ≥ 440 ms in men and≥ 460 ms in women (RR - 1.20, p = 0.03) and signs of LVH (RR- 1.16; p = 0, 004).

Conclusions. The results of this population-based study showed that the presence SRRZH in the bottom leads increases the risk of death from cardiac and arrhythmic causes. How SRRZH increases the risk of adverse outcomes is not established. According to the experimental data, the elevation of the point J is a marker of increased transmural in homogeneity of ventricular repolarization and may reflect a predisposition to ventricular tachyarrhythmias. "Future clinical and experimental studies should focus on clarifying the exact causes and mechanisms of development SRRZH and, ultimately, to develop strategies to prevent premature death from cardiac causes in patients with this disorder EKG" - the authors conclude the publication.

Psoriasis: Topical treatment

One of the most popular in the world of inflammatory chronic disease of the skin is psoriasis. On average, according to various sources, about 2.5 percent of people worldwide suffer from this disease, regardless of where they live, what their social status, gender and skin color. Among dermatological patients more than five percent of people suffer from psoriasis. Symptoms occur most often around the age of 28 years, with 10-15 percent of cases - children whose age is less than ten years. In Europe, psoriasis is sick about five million people. It is worth noting that in the last ten years have seen a significant increase in the number of people infected with psoriasis young age. In this case, notes the increasing proportion of patients with severe disabling diseases that are resistant to psoriatic arthropathy, psoriatic erythroderma, and other traditional methods of treatment of psoriasis. And about ten percent of the patients recorded psoriatic arthritis, which, in essence, is a complication of psoriasis.

To date, the pathogenesis and etiology of psoriasis are practically unknown, are held constant research on the disease, put forward many theories of its formation and development. Generally accepted, but not universally accepted, are a few theories - metabolic (exchange), neurogenic, genetic (hereditary) and viral. In this regard, each physician treats the disease in its own way, guided by the traditions of a particular theory of therapy. Most likely, the basis of psoriasis is multifactorial nature. Some importance in the pathogenesis of psoriasis is given enzymatic, immunological and other biochemical disorders of the body.

The main manifestation of pathology in psoriasis is epidermal hyperproliferation. Based on this, and try to determine its mechanisms, which will explore the pathogenesis of the disease and develop effective therapies that can help cure millions of people around the globe. T lymphocytes are the cellular elements of the dermal infiltrates, and B-lymphocytes are recorded in separate histological means in the form of single cells. Most T-lymphocytes that infiltrated the dermis, represent a T-helper cell. In this case, together with T-lymphocytes in the dermis (as it infiltrates) noted the presence of cells and Langernsa micro-failure monocytic cells.

The accumulated experience of therapy, different studies have revealed the presence of various functional disorders of the peripheral and central nervous system in people suffering from psoriasis. In addition, patients with psoriasis marked decrease in adrenal glucocorticoid function, disorders of the functionality of the pancreas, liver and other organs of the digestive disorders of lipid, protein, carbohydrate, water and salt, nitrogen, and other types of metabolism. In people with psoriasis and their relatives by blood fixed ultrastructural changes of the same type of pathological nature. Furthermore, these changes begin to appear much earlier than observed in clinical signs of disease. Predisposition of hereditary type, usually manifests itself under the influence of triggering factors trigger the body, for example, the focal infection foci, which promote autoimmunization. As you can see, we can say with certainty that psoriasis is a systemic multifactor disease pathogenesis which is largely dependent on immunological disorders, such as from dermatosis immuno dependent.

Psoriasis occurs over a long period of time in the chronic form of remission and exacerbation in general, to distinguish three stages of the disease - retrogressive, steady and progressive. As a rule, marked seasonality of the disease. Winter and autumn recorded recurrences of the disease in "winter" types of patients with improvement of the clinical picture in the summer. The "old" types of patients the disease involves deterioration in the summer. At the stage of progression of papular eruptions occur and are actively seeking to peripheral growth. Typically, at this stage, scales are covered only centers rash, but the peripheral zones are different so-called "halo of growth" - a free area of the flakes in the shape of the corolla red. The stationary phase, the corolla growth "is not marked, and new eruptions are not recorded.

Diagnosing psoriasis is based on the clinical picture. Differential diagnosis is carried out with a flat red lichen, syphiloderm papular. Certain problems arise in differentiating arthropathic psoriasis with arthritis rheumatoid. The clinical picture of symptoms is usually expressed in the formation of a monomorphic papular rash of rashes, which have a diameter of several millimeters to 1-2 centimeters. Localization of elements occurs in the hairy area of the head on the trunk, on the surfaces of straightening the arms and legs (most often - in the areas of knee and elbow joints). Rarely affected person, soles of the feet and the palms. Pinkish-red papules covered with scales white-silver, slightly above the skin surface are issued, have rounded contours scraping scales showing three diagnostic feature the first - a blood dew (there bleeding point type for scraping scales). The second - a terminal film (recorded a brilliant wet surface of reddish color under the scales, which are separated). The third criterion - stearic acid stains in this case, the scales fallen from papulnyh surfaces.

Papules psoriatic type had a significant tendency to increase the eccentric, causing the formation of plaques; their diameter is equal to several centimeters (3 to 100). Then there is a merger of plaques in the affected area of the continuous type, which are characterized by a palm-sized and irregular contour. Around papules on the progression of the disease is fixed ring of bright-red shade, which did not cover flakes. This proves the eccentricity growth papules. Are typical, and multiple new lesions, isomorphic to a positive reaction (a phenomenon Kebnera) manifests itself in a period from one week to ten days, and there is also a subjective itch? Plaque dissolved slowly in the devolution process, and the infiltration of the papules and the color intensity decreases. Plaques begin to dissolve either from the peripheral region, or from the central zone (in this case, the rash becomes ring-shaped). Papules around the rim is formed by Voronov (pseudo atrophy chesky) - depigmented white corolla. At places where there was resorption appears pseudo glee koderma - depigmentation of the time. At remission, in the areas of knee and elbow joints are marked individual (so-called "duty") plaque. A similar type of disease does not derail the disabled person, and recognized a small cosmetic flaw. In other cases, the disease more severe, there is no remission, there is a functional disorder of the kidneys (psoriatic erythroderma), an increase of peripheral lymph nodes in this case, the defeat spread over large skin areas.

Forms of psoriasis

Psoriasis is made to subdivide into several forms, which depend on the clinical features of the process.

1. Patients with diabetes often formed exudative form of the disease. It is characterized by bright and swollen with psoriatic papules on the surface of which formed a yellowish crust, like scales (since they are soaked with exudate a major form of expression of small-sized cavity exudate with a sterile form. This form is called pustular psoriasis, and it is considered to be the severe type of disease.

2. Palmar-plantar type psoriasis lesions seen the soles and palms. Their skin covered with scales and cracks, grubeyut and blush. Psoriatic plaques, particularly evident to the edges of soles and palms. Palmar-plantar type of psoriasis observed in 10 percent of people with psoriasis.

3. Seborrheic form of psoriasis strikes, usually hairy areas on his head.
In addition, it isolated warty and eczematous form of the disease. Sometimes seeing defeat nail plates, in this case, the lesion is the twofold form. Usually, the plate having dobney copolymerization of yam point depressions, and nail plate resembles a thimble. This phenomenon is called the syndrome of a thimble. In some cases, it formed hyponychial hyperkeratosis, which is characterized by a dirty-yellow nail plate. In this case, the nail thickens considerably (oni hogrifoz) or crumbling (psoriatic nail dystrophy).

Psoriasis Treatment

To date, treatment of psoriasis is a fairly simple process. In some situations, therapy can be very successful, as there are many variants of therapeutic effects, but there is merely a temporary effect of the therapy. Only in rare cases there is the improvement in quality of life of patients who record the duration of remission. It is worth noting that the significant role played by motivation of the patient, his social and family circumstances. Some of the patients satisfied with even minor improvements in the papule common type, and part want to achieve full and rapid recovery of the smallest manifestations of the disease. Strategy for treatment of people with psoriasis is compiled with the orientation of the multiple indicators. In this case, be sure to consider and adopt the following postulates:

• Therapy aims to achieve lasting remission.
• To maintain the confidence of the patient to the treatment process must be achieved as quickly as possible the first results of therapy.
• A local external therapy should be based on the conventional elements of a case, whose beneficial effects documented in vivo.

Methods and regimen are very diverse, as mentioned earlier in this article, and usually include a drug treatment, PUVA-therapy and phototherapy, exterior treatment, climatotherapy and various alternative methods. It makes sense to elaborate on the methods of treatment, as each of them can lead to many complications. And the manifestations of these complications are quite diverse, there may be a simple and itching or vomiting, and severe skin cancer lesions or functional disorders of the kidneys. The main drawback of the same therapy for psoriasis is the inability to complete cure a person from the disease. In this case, as such, the goal of completely cure psoriasis modern medicine and not posed. Success is the reduction of psoriasis to the smallest manifestations of the disease.

To start to dwell on the medicinal treatment of psoriasis. Overall treatment technology is not there; in each case, your doctor selects the regimen individually. It is very important is the fact that patients are not having any idea about the nature of the disease can literally "get stuck" on the disease, which leads to frustration and disappointment in therapy. And this although very often from the attending physician is required to use different combinations of drugs that are matched precisely the experimental method. In general, the treatment of psoriasis should be based on such factors as the patient's medical history, severity or extent of disease, type "suitability" of a person to therapy, and his lifestyle, sex and age of the patient-specific characteristics (for example, the type of skin on my knees would have required a different treatment, rather than on the face) the first stages of trying to prescribe the least "hard" drugs that have pronounced side effects. If therapy does not bring tangible results, are appointed by more sophisticated treatments are available. Already have certain side effects?

To date, very popular ointment for the treatment of psoriasis based on the calcipotriol group, for example, Dayvoneks and Psorkutan. They show the best efficiency in the treatment, ensuring in many cases, excellent treatment results. Dayvoneks used worldwide to treat psoriasis for over twenty years and more than 95 percent of patients have recovery of the clinical nature. Dayvoneks recommended for appointment as patients who have pockets of psoriasis does not exceed forty percent of the skin surface. When using calcipotriol creatine in the blood serum and the calcium content is not increased. Let's consider more in detail these drugs, given that, in principle, they are identical therapeutic agents, which are marketed under different names by distinctive manufacturers.

Daivonex

Dayvonex can initiate morphological differentiation of keratinocytes and stop their proliferation. Manufactured product as a colorless solution, which has low viscosity, a cream or ointment is used to treat psoriasis. One milliliter of the solution includes 50 micrograms calcipotriol, and it contains isopropanol, levomentol, sodium citrate and hydroxypropyl cellulose.

From the standpoint of pharmacology, Dayvoneks (the active ingredient calcipotriol) - a synthetic analogue of the metabolite of vitamin D. Calcipotriol - is a powerful repressor of T-lymphocytes, which is caused by interleukin, I. Dayvoneks indicated for use among the psoriasis vulgar, including for the treatment of his chronic forms on the scalp. Metering mode depending on what form Dayvoneks apply. For example, a day when using the ointment can be applied not more than 15 grams of the drug, while its total amount in seven days - no more than 100 g ointment applied thinly twice a day on the affected skin. The effect of using fixed usually at 7-14 days after starting therapy. The total course of treatment may be a half or two months. For pediatric patients (6-12 years), the maximum dose of ointment Dayvoneks is 50 g per week; children in the age of 12 can apply up to 75 grams of the drug for a week. A similar therapeutic scheme is used and when using the cream Dayvoneks.

Dayvoneks in the form of a solution is applied at the beginning and end of the day on the scalp, a maximum rate of 60 ml of the drug for a week.

When using Dayvoneks can see some dermatological side effects (local skin irritation, skin erythema and itching, sometimes there may be a worsening of the disease, dermatitis, including perioral its shape). And in applying the solution is likely to develop dermatitis of facial skin. From a metabolism may occur hypercalcemia and hypercalciuria. Furthermore, sometimes there are allergic reactions such as angioneurotic facial edema in rare case of a fixed-pigmentation, photosensitivity.

Contraindications to therapy Dayvoneks are diseases in which basis is abnormal calcium metabolism in the body and increased susceptibility to the components that comprise Dayvoneks. Separately, we note that the drug should be very carefully applied in hypercalciuria, hypervitaminosis D, nephrolithiasis. For children under six years of clinical data on the use of Dayvoneks as there are none, so it makes sense to be very careful to prescribe it in this category of patients. In addition, caution should be used for drug and elderly patients aged over 65 years to avoid serious side effects. Since the evidence is allocated or not Dayvoneks with breast milk available, it is undesirable to appoint him and lactating mothers during pregnancy.

When treating Dayvoneks should adhere to some basic principles. The drug is not recommended for use on skin. After treatment, arms should be washed with soap and water to eliminate the risk of getting Dayvoneks to your face. It is advisable to adhere strictly to the recommended dosage regimes, since their excess may cause hypercalcemia, which is to remove the drug. It is very important to know that when sharing solution Dayvoneks with ointment or cream, the total amount of calcipotriol per week should not be more than five milligrams. Calcipotriol in the number of five milligrams - a 40 ml solution 60 mg of a cream or ointment or 60 mL of 40 mg of ointment or cream. Dayvonek's course of therapy can be performed simultaneously with PUVA-therapy, which we describe below.

Signs of overdose Dayvoneks is to increase the calcium in the blood. In these cases, it is best to stop taking the money. Not recommended for use Dayvoneks with drugs, which have in their composition of salicylic acid.

Of great interest is the study that was conducted on patients of childhood (10-15 years), who gave a lot of useful information on the impact Dayvoneks for this age group of patients. All under the supervision of doctors in the study were 60 children, of whom identified a core group, consisting of 13 girls and 17 boys. Limitation of the disease they have ranged from 2-3 months to three years. 22 people have been diagnosed - psoriasis vulgaris in a common form (the area of the affected body surface does not exceed forty per cent), the eight - the stationary phase of limited psoriasis. The control group included 10 girls and 22 boys (4 with a diagnosis - the stationary phase of limited psoriasis and 26 - common psoriasis vulgaris). All patients in the control and study group were prescribed sedative antihistamine's vitamins B-group. Study group used Dayvoneks twice a day (children over 12 years - from 75 grams of the drug per week, up to 12 years - 50 g). In the control, group was used topically twice a day Porinden A + cream Unna. After 14 days of treatment 80% of patients (24 persons) of the main group was recorded an improvement (the size of foci decreased, the elements of pale and uplostilis), 20% of patients (six men) clinical effect in the severe form was not observed. No side effects were observed. Blood level of calcium was within normal limits. On day 14 of therapy in 93% of study group (28 persons) and 97% (29 people) of the control group was observed regression of lesions. One person from the control group and in two of the main effects of treatment were noted.

Thus, the study showed that under identical timing of therapy, a positive effect from the use of creams and ointments Porinden A Dayvoneks almost similar (97 and 93% respectively). However, it is the permanent or prolonged use of topical corticosteroids plan was unacceptable because they cause serious complications and adverse side effects. What does the preference of drugs based on calcipotriol.

Furthermore, used to treat psoriasis and drug Psorkutan in the form of 0, 005% ointment and a solution for recent use containing one gram of 50 micrograms calcipotriol. Psorcutan used as a topical therapeutic agent. The mechanism by which Psorkutan associated with the correction of the immune processes of the skin and the effect on calcium metabolism. In general, the drug almost no effect systemically, and a very small extent modifies the body calcium metabolism. Psorcutan in many cases is the best option for the treatment of psoriasis middle and mild severity.

Ointment should be applied twice a day with a thin layer on affected areas of the body, in some cases and less frequent use leads to a therapeutic effect. During the day, cannot be used for more than 15 grams of the drug, but a week - more than 100 g. The duration of therapy Psorkutan ranges from half to two, months. Ointment may appoint for a year, but remember that the system of clinical studies on this issue is not implemented. Solution of the drug rubbing light massaging movements in the skin of the head, per week, you can use up to 60 ml Psorkutan. It is worth noting that the duration of the therapeutic course depends on the complete clinical picture, and is determined individually for each patient. As well as in the case of Dayvoneks the week the total number of used calcipotriol should not exceed five milligrams (60 grams in an ointment + 30 milliliters of a solution, or 60 ml + 30 g of ointment) in the course of treatment, Psorkutan chances of adverse dermatological manifestations - papules, vesicles, pustules. Sometimes there may be a burning sensation, redness, itching, and other transient symptoms. If the product enters the skin of face the high risk of developing dermatitis, which has transmitted the character at the end of Psorkutan. It not recommended to use Psorkutan in complex disorder's functionality of the liver and kidneys, with increased sensitivity to the elements means, for pustular type of psoriasis, the progression of the disease and diseases associated with calcium metabolism. Due to lack of information on the application, Psorkutan during pregnancy and breastfeeding is not desirable to prescribe a drug of this category of patients.

Psorkutan used in combination with PUVA-therapy, as well as with other methods of therapy. Do not assign Psorkutan while taking salicylic acid. Furthermore, not recommended for use Psorkutan for patients younger than 18 years because of the lack of systematic information on treatment outcomes in this group of patients. Psorcutan drug released in the pharmacy network without a prescription, but the first makes sense to consult a physician for the possibility of its use for treatment of psoriasis.

As you can see, the drugs Dayvoneks Psorkutan and have a good performance with minimum adverse side effects, which causes their relevance in contemporary clinical practice, treatment of psoriasis.

Systematically used by the intramuscular or oral steroids are also used for treatment of psoriasis, but to say that this is a standard regimen of therapy is not feasible. Withdrawal of steroids is often associated with sudden exacerbation of the disease or with his sharp deterioration. Long-term administration of steroids can cause severe side effects. Steroid injections have no effect in lesions of multiple and can be applied only to areas of lesions, which are isolated.

Psoriasis, PUFA-therapy and rotational therapy.

As immunosuppressive agents can be assigned to methotrexate and cyclosporin. Immediately, it is worth noting that these drugs cause severe side effects. So, for example, methotrexate cause's damage to the kidneys, toxic hepatitis, thrombocytopenia, sores on the mucous membrane of the mouth, serious vomiting and nausea. In addition, the "cons" of methotrexate include the early formation of relapse after the marked improvement and torpid to other regimens subsequently. Use of methotrexate makes sense in cases where other treatment methods have failed. With widespread severe forms of psoriasis that are resistant to many types of treatment used cyclosporine. However, it is worth saying that it can be used for all patients.

And now let's focus on the treatment of psoriasis by PUFA therapy and phototherapy. On local and vast areas of damaged skin may be used UVB radiation in those situations when therapy medication does not work. Furthermore, phototherapy can be assigned in conjunction with other treatments for psoriasis. Short-term risk of the use of UVB therapy is very small, long-term - is comparable with the manifestations of solar radiation (skin aging, cancer). Nonetheless, phototherapy is recognized by many doctors one of the most effective ways to treat severe psoriasis and medium-low risk, when other methods did not reveal any effect. In recent years, many patients are trying to be treated with UVB-plants at home. It should be understood that such self-treatment is fraught with all sorts of evils, it is recommended that in any case, always consult a doctor, who will determine the intensity of the impact of UVB radiation.

PUFA-therapy is used to relieve psoriatic forms intractable to other treatments, as well as for self-treatment of strong and medium forms of psoriasis. PUFA - a drug psoralen and ultraviolet radiation, most often, this method is part of the treatment. PUFA are generally prescribed to patients who have psoriasis struck more than a third of the body surface. Emotionally, or physically debilitated patients and elderly patients may apply PUFA therapy as initial treatment with a specific use. Short-term therapy can cause reddening of the skin; they itch, nausea, and with prolonged use, PUFA may develop cataracts, premature aging of skin or the appearance of freckles. Furthermore, do not forget about the risk of basal cell carcinoma and squamous metastasizing cancer with a continued use of therapy PUFA.

Considerable preventive and therapeutic significance spa and climatic factors in psoriasis proved beneficial effects of river and sea bathing, sun exposure. Climatotherapy based on the application of natural marine products a key factor in the climate plan - this solar UVR. The most persistent effect was observed in the treatment of climatotherapy at the Dead Sea. Its high content of water of various important minerals, such as the concentration of bromine, there are 50 times more than the world's oceans, which has a calming effect on the patient's central nervous system. Typically, treatment in sanatoriums in the Dead Sea is between two weeks to a month, which is very small. It is advisable to purchase several tickets. One should know that patients with increased intensity of the disease, treatment at the Dead Sea is unacceptable as long as the process is not completely stable.

Natural sunlight cannot only weaken the psoriasis, but also significantly improve his clinical picture. Prescribed short daily dose of light of the sun, but should take care not to get sunburn, which aggravate the disease. Sunbathing to increase the risk of the formation of cancer of skin diseases, and this must not be forgotten.

Separately, talk about a rotational therapy, which has a large negative side effect, manifested in a long time, which can be reduced through the use of various methods of short-lived. The composition of rotation therapy includes cyclosporine, etretinate, methotrexate, PUFA-treatment, the resin with UVB. Each of the methods used in the treatment of rotating from one to two years, after which the patient is transferred to another method of therapy. If manifestations of psoriasis eliminated, therapy is terminated prior to the inception of disease recurrence. At relapse apply the same cycle.

There are a number of alternative treatments for psoriasis, each of which is applied with some success in various hospitals. These include - hypnosis, a method of transcranial electrical stimulation, the method of influence on the disease with extremely low temperatures, ozoterapiya, the method of microwave resonance therapy. Similar methods are used in combination with conventional treatment regimens.

According to generally accepted idea's people with psoriasis should not adhere to any restrictions on food, then there is no special diet for them is not developed. General health recommendations are warning about the need to avoid overeating, reduce consumption of foods high in animal fats and alcohol. In general, the regimen must be undemanding and easy. Therapy should be directed to the fact that the habitual way of life is not disturbed, that is unacceptable are missing work, appointments and so on. Therapy should also have a clear focus on the emotional factors of illness, so the emotional component of disease in some situations is crucial. For example, British researchers have published data that suggest that psychotherapy can explain many of the persistent manifestations of eczema, psoriasis and some other diseases of the skin. Of the 64 people suffering from chronic skin diseases, 44 patients indicated that the death of a loved one, leave, divorce, etc. and the onset of disease occurred in about the same time. That is why it is so important to consider all the psychological aspects as causes of disease and treatment of psoriasis. However, most important it is, perhaps, sincere desire to improve patient's quality of life and all possible assistance to him from family and friends.

High levels of the hormone leptin are associated with reduced risk of developing Alzheimer's disease and other dementia

These conclusions were employees of Boston University as a result of many years of observation for older Americans.

The study, which began in the 90 years of the last century, have been involved a few hundred men and women without any signs of dementia, whose average age was 79 years old.

At the first stage of the study participants measured the levels of leptin in blood plasma. This hormone, often called the "hormone of saturation, is responsible for controlling appetite and regulates the body's metabolism.

Re-examination of pensioners, including a brain MRI was conducted during 1999-2005. Finally, in 2007 was the last, the final assessment of mental abilities of participants.

Of the 198 who survived to the last survey of the elderly signs of dementia were detected in 111 patients, 89 patients had been diagnosed with Alzheimer's disease. "

Dividing the participants into four groups depending on the concentration of leptin in the blood, identified at the beginning of the study, scientists found a clear relationship between this index with the risk of all types of dementia. In the group with the greatest concentration of the hormone, this risk was reduced by 68 percent.

Of the quarter under study with the highest levels of the hormone signs of dementia were identified in 6 percent in the group with the lowest concentrations - at 25 percent. High levels of leptin were also statistically associated with a large volume of brain substance at the end of the study.

Study report published in The Journal of the American Medical Association. As the authors of the publication, if their findings can establish a more extensive research, evaluation of leptin levels in the blood may be one of the methods for assessing individual risk of developing Alzheimer's disease and other forms of dementia. In addition, based on leptin may be new methods of prevention and treatment of these diseases.

Current approaches to the treatment of gastric ulcer and duodenal ulcer

At the end of the twentieth century. Was a significant step to change the principles of treatment of peptic ulcer (BU)? The success of current approaches to therapy is associated primarily with the use of new antisecretory agents and eradication scheme's Helicobacter pylori (HP). Currently, pharmacotherapy BU includes more than 500 different products and around 1000 combinations. The modern concept of treating a BU provides active therapeutic tactics, including multi-drug schemes and long-term medications may be needed an important component of modern drug therapy of peptic ulcer - a lack of fundamental differences in approaches to the treatment of gastric ulcers and duodenal ulcers.

The main principles of therapy of peptic ulcer disease are:
  • impact on the factors of aggression and / or protection;
  • Etiological therapy;
  • adjustment of medication, taking into account comorbidity;
  • individual patient characteristics (age, body weight, tolerance to medications used, the activity,
  • i.e., the ability to serve themselves);
  • financial capacity of the patient.
  • The main directions in the treatment of peptic ulcer during the exacerbation include:
  • etiological treatment;
  • medical treatment;
  • clinical nutrition;
  • medication;
  • herbal medicine;
  • use of mineral waters;
  • physiotherapy;
  • local treatment for a long time not healing ulcers.
Currently, in the pathogenesis of ulcer, especially duodenal ulcers, attaches great importance to the infectious agent - H. pylori. Epidemiological data from different countries, indicate that 100% of duodenal ulcers and more than 80% of ulcers with localization in the stomach associated with persistence of HP.

Many studies have confirmed that H. pylori treatment reduces the recurrence of gastric ulcer (GU) and duodenal ulcer (DU). The strategy of treating a BU with eradication of HP infection has a distinct advantage over all treatment groups, anti ulcer drugs, as it provides long-term remission of the disease, and possibly a complete cure. H. pylori therapy has been well studied in accordance with the standards of evidence-based medicine Current approaches to diagnosis and treatment of infection H. pylori, meeting the requirements of evidence-based medicine is reflected in the outcome document of the second Maastricht Consensus, adopted in September 2000, the main difference between this instrument of agreement five years ago was a few important points.

  • For the first time to treat the infection H. pylori, and hence the associated diseases charged with the responsibilities of a general practitioner rather than a specialist, a gastroenterologist, as it was accepted earlier. The competence of the gastroenterologist assigned only those cases where treatment of the disease, including using a second-line therapy, was unsuccessful, as well as cases explicitly require intervention specialist.
  • Introduced a two-stage treatment: choosing a first-line regimens, a doctor at once, should plan a backup therapy.
  • It is recommended to use H. pylori therapy in patients with functional dyspepsia, as well as in cases where the planned long-term treatment of nonsteroidal antiinflammatory drugs.
  • Patients with uncomplicated duodenal ulcer are invited to nominate only the recommended courses of Helicobacter therapy, without the subsequent use of antisecretory drugs.
  • The main criterion for selection Helicobacter therapy is its expected performance, providing a high percentage of eradication (80%) rules and guidelines of the Russian Association of Gastroenterology for the treatment of infections with the use of HP Helicobacter therapy.
  • If used regimen is impossible to achieve eradication; a second under this scheme should not be.
  • If you used the scheme has not led to eradication; it means that the bacterium has acquired resistance to one component of treatment regimens.
  • If you use one, then another treatment regimen does not lead to eradication; it is necessary to determine the sensitivity of the strain of HP to the whole spectrum of antibiotics used.

Adoption of the Russian Gastroenterological Association in 1998, national guidelines for diagnosis and treatment of Helicobacter pylori infection and the massive introduction of these doctors have thus far failed to reduce the number of strategic and tactical errors in determining the indications for eradication of H. pylori and the choice of appropriate schemes (see table 1) .

Table 1. Errors in the treatment of HP infection.

What you need to know your doctor is starting Helicobacter therapy? Each general practitioner, particularly with experience of more than five years, will likely have to overcome a psychological barrier before appoint a patient with peptic ulcer disease with antibiotics. To date, doctors, gastroenterologists and internists still in many ways related to Helicobacter therapy at BU. Must be the strict observance scheme H. pylori treatment.

Their effectiveness is proven; they correspond to features of HP and pharmacokinetics of drugs are also known side effects of such therapy.

It is better not to H. pylori therapy in general, rather than spend it wrong - as in this case, quickly develop resistance to a variety of HP components. In this regard, the patient must give details about the upcoming treatment and to seek his agreement to cooperate with the doctor. It is also important to evaluate the possibility of patient material. He should know that, thanks to be expensive, single-treatment can achieve stable remission in patients with duodenal ulcer in 70-80% of cases, while at GU - 50-60%, which ultimately cost effective.

What is the eradication scheme to choose? If you have a stomach ulcer or duodenal against the backdrop of increased acid production, preference should be given to the classic three-part scheme based on the blocker, proton pump (PPI) (omeprazole, etc.) then a transition to the single-dose PPI without antibiotics. Do not use the schema containing Nitroimidazoles (metronidazole, tinidazole), if the history of this group of drugs administered for other indications.

Currently, Russia has witnessed a sharp increase in the number of HP strains that are resistant to nitro imidazole. In view of this urgent problem today is the search for more efficient modes of HP eradication therefore, in recent years a growing interest in the use of macrolides in the treatment of HF-associated diseases. Numerous work has shown the effectiveness of macrolide antibiotics for the treatment of HF. These drugs have a high ability to penetrate cells, intensely highlighted by the mucous membranes (JI), which increases their effectiveness against HP. In addition, macrolide antibiotics, fewer contraindications and side effects, they have a higher rate of eradication than tetracycline, which can also accumulate in the cells. A feature of HP infection is that it is accompanied by giperatsid playin.

In this regard, the majority of macrolide antibiotics, exposed to enhanced hydrolysis and cannot be used. An exception is the clarithromycin resistant to hydrochloric acid.

Therefore, the aim of our study was the development of new schemes of eradication therapy of duodenal ulcer associated with H. pylori, with the use of omeprazole (O), as well as the combination of amoxicillin (A) and clarithromycin (C). We used the following scheme for eradication - ultop (omeprazole) 20 mg twice daily + fromilid (clarithromycin) 500 mg twice daily + hikontsil (amoxicillin) 1000 mg twice a day - a course for seven days. Eradication was 90%. The study showed that the use of fromilida (clarithromycin) to effectively and expediently in the schemes of Helicobacter therapy with PPIs.

These numerous studies and the results of their meta-analysis led to the conclusion that the inclusion of antisecretory drugs in the scheme of eradication of HP-infection not only enhances the eradication of HP in combination with antibiotics, but also accelerates scarring sores makes it easier to eliminate the symptoms of ulcer dyspepsia. As for specific mechanisms to improve eradication rates by taking antisecretory drugs, first of all, with an increase in pH of gastric contents is reduced rate of minimum inhibitory concentration of antibiotics (MIC) and accordingly increases their effectiveness. Furthermore, increase the viscosity of gastric juice and concentration of antibiotic in the gastric contents, thus increasing the exposure time of antibacterial drugs with the bacterium H. pylori. We have investigated the effectiveness of ultopa (omeprazole) - pH> 4 in the gastric single dose of 20 mg has been around for 12-14 h (see Figure 1).

However, the PPI of the first generation did not fully meet the practical needs of clinicians. They are slowly converted into an active form and create the maximum for the eradication antisecretory effect only for the fifth-eighth day of therapy. Among the drugs in this class also includes lansoprazole, pantoprazole, rabeprazole and esomeprazole. They bind to the enzymes of the cell wall of parietal cells - H +, K +-ATPase, and are the most powerful means of controlling gastric acid production.

With the help of pH-metry in HP-negative volunteers studied the action of a new dosage form Loseke ITA. After treatment with this drug antisecretory effect during the day was even more pronounced than with pantoprazole. However, pharmaceutical companies continue to pursue brand new, more effective antisecretory funds, created a new product - Nexium. Antisecretory effect of Nexium is superior to severity, rapidity of onset and duration of exposure such as the effect of omeprazole at standard doses of 20 and 40 mg, pantoprazole 40 mg and lansoprazole 30 mg.

In connection with the above, great interest is the new PPI - Pariet (rabeprazole). In the treatment of gastric ulcer and duodenal recommended taking Pariet 40 mg once daily or 20 mg every 12 h. The most effective, rapid antisecretory and antibacterial agent in the schemes of eradication is Pariet 20 mg twice a day. It should not be appointed within seven days before the start of antibiotic treatment, as in the case of other STIs as well as a reliable antisecretory effect is achieved on the first day of treatment (from the recommendations of the Gastroenterological Association).

Unfortunately, the presence of some patients resistant to antibiotic's HP forcing researchers to develop alternative treatment options for patients suffering from peptic ulcer associated with H. pylori.

Thus, we studied the effectiveness of eradication schemes using redundant antibacterial drugs. The best result of eradication (90%) was achieved using the scheme: De-nol 240 mg twice a day, 14 days + tetracycline 1 g / day, and furazolidone 200 mg twice a day, seven days.

Quite often, the question arises about the need for eradication therapy in elderly and senile age. To date, this can be attributed to the fact that the long-term persistence of HP is developing intestinal metaplasia and atrophy of the gastric mucosa, increases the risk of gastric carcinoma. Age peculiarities of enzymatic activity and atrophic processes in the SB of the gastrointestinal tract and alter the rate of biotransformation of drugs in violation of their absorption. It noted that the concentration of ranitidine is increased in patients over 60 years with comorbidity hepato pancreato biliar region.

"Ahilesovoy heel" of conservative treatment of peptic ulcer disease is well known, the high rate of complications. We prove that eradication of HP completely prevents complications of peptic ulcer. So, in four large studies investigated during peptic ulcer disease in patients in which it manifested bleeding. As I can be seen from the data, any other kind of treatment does not exclude the danger of re-bleeding - within one year after the previous hemorrhage, it recurs in approximately every third patient. In the case of eradication NP bleeding did not recur.

The relapse rate of bleeding after a course of eradication therapy.
Evaluating the effectiveness of eradication is carried out after completion of treatment and aims to identify vegetative and coccoid forms of H. pylori. In the "Recommendations" is clearly defined scheme of this phase of diagnosis:

  • dates - no earlier than four to six weeks after the course protiv helicobacter noy therapy or after treatment of comorbidities any antibiotics or antisecretory means;
  • diagnosis of eradication by using at least two of these diagnostic methods, and using methods that make possible the direct detection of bacteria in biopsy material (bacteriological, histological, urease) necessary to study the two biopsies from the gastric body and a biopsy of the antrum.

You cannot underestimate the role of antacids in the treatment of gastric ulcer and duodenum these drugs, known since ancient times, lower the acidity of gastric juice by chemical interaction with the acid in the stomach. Preference will be given antacids are not absorbed - almagel, Maalox, Aluminium phosphate gel, taltsid, rutatsid. During exacerbation of gastric ulcer and duodenum in treatment, we used rutatsid 500 mg three times a day plus one tablet before bedtime. While taking this remedy the symptoms of gastric dyspepsia disappeared
by the end of the first and second days of treatment. Despite the introduction of modern medical practice inhibitors of gastric secretion, antacids remain important as an effective means of treating patients with gastric ulcer and duodenum.

The treatment must be achieved complete clinical and endoscopic remission, with negative results of the HP-test.

It should be noted that we rarely find cases where the patient has an isolated ulcer. Treatment of comorbidity associated with a number of problems.

Sometimes the conservative therapy is ineffective. This may be due to two factors: the often recurrent course of peptic ulcer and the formation of refractory gastroduodenal ulcers. The analysis revealed the reasons for frequent recurrences within BU, it is - HP infection, use of nonsteroidal anti-inflammatory drugs, the presence of a history of ulcer complications, and low compliance. The factors contributing to the formation of refractory gastroduodenal ulcers, may make the above reasons, as well as latent Zollinger-Ellison syndrome.

In conclusion, we reiterate the critical importance of developing national standards for treatment of gastric ulcer and duodenum and their rapid implementation in practice of a doctor - physician and gastroenterologist. Important arguments in favor of H. pylori treatment were obtained when evaluating the ratio cost / effectiveness. BU is widespread and is characterized by chronic recurrent course. Eradication of H. pylori reduces both direct and indirect costs at BU, without the need for costly maintenance treatment, antisecretory drugs, the risk of repeated exacerbations, complications, and in some cases, surgery.

Thus, modern drug therapy duodenal ulcer and gastric ulcer may provide disease-free for these diseases and to spare patients from complications. In most cases, outpatient treatment is sufficient. The success of therapy depends not only on the optimal use of drug combinations, but also to a large extent, on its implementation with the participation of the patient.

Change of antipsychotic drugs is the common practice in the treatment of mental disorders. Scientists from the laboratories of the company Eli Lilly & Co. Conducted a study whose aim was to determine the clinical and economic implications of changing antipsychotic drugs for outpatient treatment of schizophrenia - tells the magazine BMC Psychiatry.

As part of a one-year randomized study examined the use of typical and atypical antipsychotics. The study protocol meant changing from one antipsychotic to another only if it was justified. Among outpatients, who were replaced by drugs, was determined by the risk of a sharp deterioration in health status, treatment for emergency medical assistance, as well as the time period prior to such events. These figures were compared with those among patients who continued to take first-line drugs. Data were obtained from a study of medical records and by interviewing the participants themselves.

The results showed that compared with patients who continued taking the drug was originally designated; patients changed their antipsychotic, has been elevated risk of hospitalization and emergency medical intervention. In addition, patients changed their antipsychotic medication, spend an average of 3000 dollars USA (25% of total) per annum more than not changing the drug. Most of these funds accounted for the already mentioned cases of emergency medical care.

Based on the results, the authors concluded that a change of antipsychotic drugs is associated with worse clinical and economic consequences, resulting in more frequent requests for emergency aid.

Statins: the most important

Statins or reductase inhibitors of 3-hydroxy-3-methylglutaryl coenzyme An entered into clinical practice in the 80s of last century as a major lipid-lowering drug.

It later emerged that they also possess anti-inflammatory action, and because in the end triumphed over inflammatory hypothesis of atherosclerosis had become major in its treatment, from primary prevention and ending with all the variety of complications.

As a result, clinical use of statins reduced cardiovascular and total mortality, improves quality of life and prognosis for patients with any manifestation of atherosclerosis.

It is not contested that the long-term (two years) continuous treatment with statins slows the progression of atherosclerosis. The first phase of treatment with statins lipid-lowering therapy was in its true sense to target cholesterol levels. It gradually changed the paradigm of treatment, when they began to understand that atherosclerosis may be at a low level of cholesterol. In severe clinical atherosclerosis (coronary, cerebral, either), even with low-cholesterol high-dose statins exert their therapeutic effect and in this have no alternative. With anti-inflammatory effects of statins associated decrease in volume and increase the elasticity of the lipid core with improved properties of the connective tissue sheath of atherosclerotic plaques, slowing down, and in some cases the reverse of their development, improvement of endothelial function, decrease in inflammatory activity with a decrease in the thickness of the intima and media of arteries of muscular and musculocutaneous elastic-type, etc. Statins have a modulating effect on inflammation, with decreases blood levels of C-reactive protein, circulating biochemical markers of inflammation (soluble forms of P-and E-selectin and intercellular adhesion molecules (ICAM-1)) and hemostatic ( tissue plasminogen activator antigen (tPAag), Villebranta factor and thrombomodulin) activity of the endothelium.

Two-pronged hypolipidemic and anti-inflammatory mechanism of action of statins causes a lot of positive clinical effects, at first glance surprising, because these pleiotropic among them? Improvement of cognitive functions, reducing the number and variety of arrhythmias, improve cardiac pump function, mochi brazovatelnoy renal function, reduction in systolic and diastolic blood pressure, improve glucose metabolism in diabetes, even just improving your general health, and many others. These effects are caused by direct action of statins comparable to the hypolipidemic effect, and their assignment to the pleiotropic is purely historical. Moreover, they do not depend on the hypolipidemic effect of the drug.

The main pharmacodynamic effects of statins
  • Dilation and prevention of arterial spasm
  • Stimulation of angiogenesis
  • inhibition of proliferation of vascular smooth muscle cells
  • slowing down the formation, development and stabilization of atherosclerotic plaques
  • inhibition of platelet aggregation, decrease platelet and red blood cells with a decrease in blood viscosity
  • reducing the level of thrombin and stimulation of fibrinolysis, the decrease in the level of the plasminogen activator inhibitors
  • increase the synthesis of endothelial
  • inhibition of macrophage migration into the vascular wall
  • antioxidant effect
  • immunosuppressive effect
  • reduction of left ventricular hypertrophy
  • antiarrhythmic effect
  • reduction of albuminuria in diabetes mellitus
  • slowing the progression of Alzheimer's disease and dementia
